Welcome to the Cannon Air Force Base security clearance form. This form must be completed for all event attendees age 18 and older. Before you begin the form, please have the home address and driver's license or passport number ready for each guest in your party. After you complete the form, you will receive a confirmation email. After completion of the form, you may purchase tickets.
This form can be completed for additional guests in your party at a later date if needed, but the form must be completed before arriving on base for Wings Over Cannon.
